For your convenience, we accept cash, checks, and most major credit cards. Payment is expected at the time services are performed.

Insurance

In continuing service to our patients, Dr. Ellen accepts most insurance plans. Our staff will help you prepare all the necessary forms for your medical benefits provider; however, we remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.

We will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be before you schedule your procedure.

The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should also be aware that all insurance companies vary greatly in the types of coverage available.

Financing

We are pleased to offer financing options through CareCredit® and Chase Health Advance.

CareCredit® can provide you with a financing option that can help you by making your procedure more affordable. After your initial consultation, we would be happy to develop a treatment plan tailored to your specific needs. Once your treatment plan has been finalized, you may apply for financing through CareCredit®.
